Wrexham Health, Social Care and Well-being Partnership
The National Health Service (Wales) Act 2006 places a duty on Local Health Boards and Local Authorities to jointly formulate and implement a stratgy for the health, social care and well-being of its local population. The Wrexham Health, Social Care and Well-being Partnership Board, the membership of which is made up of senior representatives from Wrexham Local Health Board, Wrexham County Borough Council, North Wales NHS Trust (Eastern), National Public Health Service, Clwyd Community Health Council and Association of Voluntary Organisations in Wrexham.
Health, Social Care and Well-being is not just about our traditional perceptions of a health care service or a local authority that provides social support, health, social care and well-being is about ensuring the full potential in every aspect of our daily lives is met. Our everyday needs are interdependent and it is essential to plan for all such needs and not just those related to being ill or requiring care.
"Caring for our Health 2008 - 2011" is the health, social care and well-being strategy for Wrexham. It is based on a sound Needs Assessment - this process involved collecting and analysing population data and health trends, but also gathering qualitive information about peoples perceptions and experiences. The overall vision is:
"To improve the health, social inclusion and well-being of children, young people and adults"
With four key aims:
- To improve the health and well-being of children, young people and adults
- To improve access to health, social care and well-being services
- To improve the quality of health, social care and well-being services
- To improve citizen engagement and satisfaction with health, social care and well-being services
Underpinning the aims are a range of service areas and specific outcomes to be achieved. The Health, Social Care and Well-being Partnership Board has established a range of Commissioning Groups and Programme Groups to take forward the actions and work programmes.
The Voluntary Sector is represented at all levels of the planning structures. AVOW facilitates a Health, Social Care and Well-being (Adults) Forum, and published regular electronic bullitins and the Health, Social Care and Well-being Issues Newsletter to ensure that the local voluntary sector is fully engaged in the planning processes.
